Sample Kit Order Form
Use this form to order DNA hair sample kits for your purebred Arabian breeding
stock. Mail this form with payment to the Canadian Arabian Horse Registry office at the
above address. Payment can be made by CERTIFIED cheque, money order, Visa or MasterCard.
As of January 1, 2001 PERSONAL CHEQUES are no longer being accepted. If paying by credit
card, faxes are acceptable. The applicable fee must be paid before the kit(s) will be
mailed.
NOTE: Please do not send hair to the CAHR Office. Once paid for your kit will be mailed to you, at which time you will affix the hair as instructed to the appropriate section on the form provided. The DNA kit will then be sent by you directly to the Lab for processing. A pre-addressed envelope is included with the kit.
