Stored Semen Service Certificate
I hereby acknowledge that I have read and agree to be bound by the Registry's Rules and Regulations established for transported/stored semen. I also understand that non-compliance with these rules and regulations will be investigated by the Registry, any resultant foals may be ineligible for registration and the penalties as provided for under Articles 19 & 26 of the CAHR Constitution may be imposed.
I understand that the stallion must have a valid Transported/Stored Semen Permit before service certificates will be issued. I am aware that one service certificate is required to register a foal conceived using semen which has been transported or stored as described in the Registry's Rules and Regulations.
I have enclosed the applicable fee for each service certificate requested for the stallion listed below:
