I hereby acknowledge that I have read and agree to be bound by the Registry's Rules and Regulations established for transported/stored semen. I also understand that non-compliance with these rules and regulations will be investigated by the Registry, any resultant foals may be ineligible for registration and the penalties as provided for under Articles 19 & 26 of the CAHR Constitution may be imposed. I understand the DNA type of the stallion must be on permanent record with the Registry before the permit will be issued. I am aware that collecting, transporting or freezing semen for use as described in the Registry's Rules and Regulations governing Artificial Insemination Using Transported or Stored Semen prior to the issuance of a permit will result in an assessment of a Late Permit Fee. I have enclosed the applicable permit fee and hereby request a Transported/Stored Semen Permit for the stallion listed below:
